This Rebate Form must be received by SpeedyDirect before April 30, 2005.
SpeedyDirect is not responsible for lost or stolen rebate forms.
Please allow 7 to 10 weeks to receive your Rebate Check from SpeedyDirect.
You must be an active DIRECTV subscriber at the time this form is submitted.
